Hello, Dr. Pelto. Here. Welcome to the central Massachusetts podiatry newsletter for November 2021.
Today, we have a lot of stuff that I have a special guest for you guys that stick around. He's sitting here in the room with me. All about big toe pain is the webinar, you can either register for it, we're doing the 33 you can get the replay. With that same link, Doctor Saviet it and myself. We both did some interviews with concierge physical therapy. The diagnosis of the month we're gonna talk about diabetes because November is Diabetes Awareness Month. We have these new shoe horns, if you guys want one, you can just ask for one when you're in the office. If you don't watch the video, you're probably not going to know to ask, and then a question and answer. We're going to answer that. And then I have something just for laughs. If patients tell me something funny, I like to share it with others. So let's get into it.

Diagnosis of the month we're talking about diabetes. Why? Because November is Diabetes Awareness Month. So the one thing I would say if you are diabetic or you have a loved one that's diabetic, I would see you soon you want to see an eye doctor once a year. To look at your eyes. You should see your endocrinologist or diabetes doctor, you should see like a dentist because your teeth get affected and you should get a diabetic foot exam, a comprehensive diabetic foot exam. If you've never had one of those. It's a wonderful experience, okay, you get your foot exam, we look at the pulses, we look at the nerves and everything like that. You could if you qualify, get diabetic shoes, and we have this new company we're using where they go right to your house. And then you should be checking your feet every day. I'm putting a link underneath here. There's this little course I just call it a course it's not really a course it's it's a whole bunch of links to good stuff for people that have diabetes and a book. I'm going to put a link underneath there if you want to get that.
This is the free shoehorn isn't this beautiful? Right? It's just because all of our patients were asking for our shoehorns, and I didn't want to give her I couldn't give them to him. You know, the Hey, we like to shoehorn in and I said, Well, maybe we should just get shoehorn. So we ordered a whole bunch of these. And if you want one, just ask the office, they work great. I think the real longer ones are better. There's a real long ones, but they wouldn't fit in our treatment rooms. So this one's pretty good. So tell me what you think if you want one, okay. You can have to come in to get that you can't just send an email to me. I can't email you a shoehorn.
This is the kind of the product of the month. This is something cool. It's called a doff n donner. I don't know if you've ever heard of this. But what a Doff N Donner is, it is something for people that wear compression stockings and they can't get them on. And what a what it is, is basically it's, it's you put the compression stocking on, you can see the heel of the compression stocking, you put this little tube and you wrap that thing up and it just slides right on. It's about $100 on Amazon, I put a link underneath it, but it is for people that can't get their compression stockings because everyone buys them and no one can put them on they try. They try gloves, they try you know other things others devices, but this is the best one that I found so far. I'd like to know what you think about that. You can just comment underneath this video.
And now our special guest, Dr. Kllner Come on over. So Dr. Kellner , okay. Hey, how are you? Oh, good. Good. We have seen it's not going to be just the Don show anymore. It's gonna be the down and Sam show when you're here whenever it's already such. So here I tell the audience true false. You can't do anything for a toll break or fracture. There are two examples here, one on the left one on the right.

I definitely select My answer is false. Definitely look at the x-ray here on the very left, this person fractured their pinky toe, and their toe was now turned outward. So that toe should actually absolutely be evaluated, which would definitely need to be reduced, which is realigning the bones back into place, which is something we do in the office after numbing it up and repositioning it. And then the other x rays here certainly shouldn't be. That's a pronoun dry thing. Well, that's before that's the reduction there. So that's also this picture on the right. Yeah, reduction. That's what it should look like after the bones have been repositioned? In better alignment, where should be you don't want your pinky toe stuck pointing outwards. And then the other fracture

that was a patient of mine that came in like to me I say, Well, what can you do? Oh, nothing. This was two months later.

Before After, okay, great. Before is on the left here. Yep. Okay, so the fracture actually, on the new did or gotten multiple pieces through the joint and got a little bit worse, still would let that heal in its position. But they're certainly going to have arthritis and then join later in life and whether that becomes a problem or not, should be addressed maybe with surgery or without surgery as needed. in what Afrikaner when does someone need to come? Is it everyone that stubbed their toe at nighttime? Or are there certain instances? What do you look for
Dr. Sam Kellner
Hi, I would definitely look for significant bruising, swelling, pain, or especially dislocation, like that first X-ray there that those should definitely be evaluated, especially if it's not getting better than once in a week and

Should they like tape it or use something else?

If they can tolerate any sugar, I definitely recommend what's called a post-op shoe. It's a very stiff-soled sandal that can help them walk a little bit better doesn't allow the toe to bend when you're walking in that sometimes when there's an angulated fracture and the toe is going maybe outward this way,

as the best, I can do my fingers there. But say that the pinky toe was going out this way I want to body splint it to the adjacent toe so that holds in a better position. But if the toes are in a good position, I usually just kind of do a straight wrapping around the toe itself. So compression sleeves help with the swelling and holding industry position.

As I finish up here, just for fun, this was a patient of mine, the patient new I'm from Minnesota, and we did this this video it's our old one from 2009 called new in town. And I went and just watch the and whatnot and watch the whole thing and I want to it's called new in town. But this is a if you don't know Minnesota Vikings so here this is what everyone does. They have the hats, the Viking hats, they have the Viking uniforms. What they're missing is the brick. What we have is the sponge bricks, the styrofoam of sponge bricks in Minnesota, that that we use to throw out the TV when our team doesn't win. So anyway, that's I'm from Minnesota.
