				<content:encoded><![CDATA[<b style="mso-bidi-font-weight: normal;"><span style="font-size: 10.0pt;">What shoes does a podiatrist wear when running?</span></b><br /><div class="MsoNoSpacing"><br /></div><div class="MsoNoSpacing"><span style="font-size: 10.0pt;">I get a lot of questions from my patients in Worcester, MA about what type of shoes I wear when I like to run. There is a local store called Sneakerama in Worcester that I buy my sneakers from and the one that I like to use most recently are called Asics Gel Kayano. These are shoes that have very good support and they have something called biased lacing. What that means is that instead of the laces being straight up and down the foot they are more off to the right-hand side or to the medial side of your foot. This reduces any of the impingement on the big toe joint when you are running. My basic tips for shoes is I like to change them every six months or every 500 miles, I like to make sure I wear new shoes frequently, and if I think they are wearing out I tend to buy a second pair and just notice the difference. If the new pair feels better I will throw out the old pair. The majority of foot problems that I see that people deal with come from wrong shoes. Some of these conditions include heel pain, Achilles tendonitis, plantar fasciitis, and even certain types of foot wounds that can occur because the shoes do not have enough support. This has information both in English and in Spanish for different types of patients. It has a wealth of information about medications, about changing your diet, and about taking care of yourself. I think you will find it very, very helpful. There are some interesting videos and especially testimonials from some other people that have diabetes. That is a very good point regarding testimonials or other people&#8217;s experiences. When someone develops a disease like diabetes they tend to isolate themselves and try to deal with it by themselves and not get anyone involved. That is why one of the best things in dealing with diabetes is to deal with a support group or a group of other people that are going through the same condition. That is why I recommend this website called TuDiabetes. 				<content:encoded><![CDATA[<div class="separator" style="clear: both; text-align: center;"><a href="http://footdokter.com/images/treatment_plantar.jpg" imageanchor="1" style="margin-left: 1em; margin-right: 1em;"><img border="0" height="213" src="http://footdokter.com/images/treatment_plantar.jpg" width="320" /></a></div><div class="separator" style="clear: both; text-align: center;"></div><div><br /></div>There are many patients in Worcester, MA that come in initially with heel pain or plantar fasciitis, but then as time goes on they develop pain on the outside or on the lateral side of the foot. Usually that is caused by compensating or a change in the way that they walk because of the heel pain so to avoid putting pressure on the center on the inside of the heel they walk more on the outside. When they do that this adjusts the way that they walk which puts more strain and pressure on the muscles, especially the peroneus brevis and peroneus longus on the outside of the leg region. The way you treat that is to deal with the heel pain on the inside of the heel with an injection, stretching or physical therapy to help reduce the pressure in that area. Once that is resolved on the inside of the heel, the outside of the heel pain should be resolved. This is a common problem, especially during the sports season when they are doing more activity or wearing unsupportive flip-flops. If this happens to your child and their foot is painful from doing activity, you may want to consider orthotics. What I usually see is that children, when they are walking normally, they are fine in any type of shoe that they wear. But as they start to increase the rate of their activity they have more pain and they need more support. You can try a good sneaker. I usually recommend patients to go to Sneakerama to find those sneakers, and then you can try an over-the-counter insert in the sneaker. But sometimes these over-the-counter inserts may be too large for a child&#8217;s foot. There are two different types of orthotics that you can try. One of the orthotics is an over-the-counter type that is based on their foot size and it increases over time and it costs about $100. This is good for a normal foot. However, this week I had a patient that had a very wide foot and we needed to get them a custom orthotic. The price is a little bit higher, around $500, but the nice thing about the custom orthotics is that you can get a pediatric outgrowth program. What that means is that just for a little bit more you can have a guarantee that within two years you can get a second pair for no cost for the orthotic (but just a office scanning fee). If your child wears orthotics and goes through a pair in two years you can get another pair, so one pair will last you for about four years. So that is a good option as well. Just so you understand, for children orthotics are needed if they have pain and fatigue when they are walking, when they are standing, and when are playing different types of activities. Foot pain is normal for a child and usually it is because they have flat feet or their tendons are pulling hard on their growth plates which also can be helped with physical therapy or some type of an orthotic device.&#160;</div>

When you come into the operating room first you are going to have a tourniquet that is going to be placed around your leg and this is going to reduce the blood flow during the course of the surgery so that things can be seen better. Your incision is going to go right on the top of your big toe joint or to the side of the toe joint depending on what your surgeon wants to do. This incision is made through the skin and it goes down to the level we call the capsule of the joint. Once you are down to the level of the capsule you need to stop the bleeding of any of the blood vessels there and then open up the area using something called a retractor. Once the area is opened up you can make an incision in the capsule. We tend to do something called an inverted L incision of the capsule. This is just the way that we cut so that we can gain access to the bump on the side of the first metatarsal head. That is the big bump that most people notice when they have a bunion. Once we free up all that skin, we can remove the bump, and then we can go to the place between the first and second bones and free up a tendon there called the conjoined tendon of the adductor hallucis. Once we free that up with a scalpel, we are able to do a cut into the head of the bone. We do a V-cut, which is the most common. This is cut straight through to allow for the head of the bone to go down and shorten a little bit based on your anatomy. Once this is done we can shift over the head of the bone so there is a little bit of a ledge on the side where the bump used to be and that allows for correction of the space between the first and second bone and also replaces the sesamoid bones underneath the head of the bone. Once this is done we put a screw in the area to hold it in place and we remove that remaining ledge that was there. The area is flushed with a lot of sterile water and stitches are used to sew up the capsule after removing a little piece of it to help readjust for the new position of the toe, and then we stitch up the skin with some other types of stitches that can hide underneath the skin. A dressing is placed on the wound usually with some iodine to serve as a splint, and then we put a different type of a wraparound there to hold it in the correct position. So these are the steps for doing bunion surgery.<o:p></o:p></div><div class="MsoNoSpacing"><br /></div><div class="MsoNoSpacing">PS.
